Starting next year, Ruby Hall Clinic will be setting up five new specialty and general hospitals over the next five years

Speaking to the media on the occasion of the 50th year of Ruby Hall Clinic, Hospital CEO Bomi Bhote said, "We will set up 100 to 150 bed specialty and general hospitals in different parts of the city. There is no more space in the existing facility and health care needs are constantly increasing.

We will be starting the hospitals on Ganeshkhind Road, Baner Road, Hinjewadi and in Wanowrie. The idea is to focus on areas where there are minimal health care facilities."

Pointing out lack of medical facilities in Hinjewadi, he said, "It is surprising to note that the master plan of the area does not have any provision for a health care facility."

He urged that FSI curbs need to be removed as there were very few hospitals in the city and there could be no logic for restrictions on FSI.

On World Organ Donor Day, November 30, a thank you for kidney donors

24 Year old Amit Jain died in a road accident last year and his mother Rekha is still trying to come to terms with the loss but she reflects with pride her dying son's decision to donate his kidneys and cornea.

On World Organ Donor Day (November 30), Amit's parents and 11 others who donated their kidney, will be felicitated at a special function here.

"There is a need for awareness on kidney donation, especially cadaver kidneys," says Rohini Sahasrabuddhe, the dialysis and transplant coordinator of the renal unit of KEM Hospital.

Sahasrabuddhe, who is also one of the trustees of the Zonal Transplant Coordination Centre, says the number of cadaver kidneys donated since 2004 in Pune region was only 24.

Rekha, whose son died last year, says, "We are happy that his kidneys have helped a patient with renal failure."

"We try to encourage relatives of patients who are brain dead to donate their organs," says Sahasrabuddhe. Efforts at times meet with success.

Recently, 40-year-old Nachiket Khisti, suffering from intra-cranial bleeding, was declared brain dead. His organs including kidneys, liver and cornea were donated. Khisti's cousin Rohan Kulkarni recalls that Nachiket had expressed a wish to donate his eyes and at KEM Hospital and counselors also convinced family members to permit donation of other organs.

Nachiket's donation saved more than four lives.

Dr F F Wadia, Director, Renal unit of KEM Hospital, where a 148 kidney transplants have been performed, says awareness is needed on kidney donation.

It was counseling that saved 40year-old Sanjay Mandlik suffering from renal failure. He had to undergo dialysis thrice a week. "For nearly four months, I was on dialysis," says Mandlik.

"Counseling at KEM's renal unit convinced my family members. Blood tests were done and my younger sister Shilpa's blood group matched. She was also married and it took a month of counseling before she was ready to donate a kidney. "Naturally, we were worried about any difficulty she may face later," says Mandlik who feels a lot stronger and healthier after the kidney transplant.

Aditya Birla Memorial Hospital (ABMH) recently launched four specialty clinics on its third anniversary recently. Chairperson of Aditya Birla Foundation Rajashree Birla inaugurated the clinics, which include a fertility centre, stroke clinic, obesity and Apne Pairon Pe clinic.

A sperm bank was also inaugurated. "Due to lifestyle changes, one out of seven couples face fertility problems. The clinic was launched to deal with such problems and to provide the latest treatment," said clinic incharge Dr Anil Chittake.

The obesity clinic will deal with medical complications like high blood pressure, diabetes, coronary artery disease, arthritis, stroke, cancer, liver cirrhosis, infertility and psychological stress.

The Apne Pairon Pe clinic will deal with the diseases caused by the obstruction of large arteries that carry blood from the heart to other parts of the body.

The hospital also launched a free bus service for its patients and their relatives. They will be picked up from places like Chakan, Bhosari, Nashik Phata, Empire Estate and Chinchwad Chowk.

"In a country where there are 30 lakh burn cases annually, the importance of a skin bank is immense. Skin regeneration as a process is expensive and takes three to four weeks to culture. In such a situation, a skin bank is invaluable," Rotarian Mukesh Agarwal said on the occasion.

Skin-grafting helps in burns and accident cases. "Human skin is the best dressing. When your skin is removed or lost due to different reasons like a burn or accident, the internal organs become susceptible. Fluids leak out, bacteria go in and nerves get exposed, resulting in severe pain. In such a case, if a donor's skin is available, it is possible to save the body from infection and pain. Through skin grafting, the donor's skin can be put over the affected area and it provides a cover while the individual's skin grows. The donor's skin falls off in three to four weeks, by which time the body has grown its own skin," explained Dr Shrirang Pandit, consultant plastic surgeon at Poona Hospital.

Dr Sunil Keswani, director, National Burns Centre, Mumbai, said,"There are two sources of skin. One is a live donor, which is the most prevalent practice across the world right now where people, usually relatives of burn victims, donate their skin. But this is a painful procedure. All this can be avoided if we have a ready skin bank, like an eye bank or blood bank, where people donate skin after death. A ready skin reduces pain and prevents loss of proteins and electrolytes."

Body skin can be donated on death. Doctors allayed fears that the body would be disfigured. "Skin is removed from the back and legs. It is a surgical procedure without bloodshed," said Dr Sunil Keswani.

Dr Welsly MJ Bodha, a surgeon from Netherlands and director, Euro Skin Bank, said, "The biggest concern is not technology, but need for public awareness and strict laws."

Cardiologists say poor dental hygiene among patients of rheumatic heart disease (RHD) increases the risk of developing endocarditisan infection of the heart caused by a bacteria. The risk of developing this infection among RHD patients is around 10 per cent, say cardiologists.

"Poor oral and dental health leads to infection of leaking valves (regurgitation) which are already affected by RHD. Such patients should maintain good oral and dental health," said cardiologist Sunil Sathe.

Now in Pune "Spitting in Public" to attract Rs 1,000 fine

"Any person found spitting in public will be fined Rs 1,000 starting September 26 till further orders. The measure is being taken because the H1N1 virus can remain alive in sputum for eight to nine hours," Municipal commissioner Mahesh Zagade

The common practice of spitting in public has finally earned strong censure from the Pune Municipal Corporation (PMC). While the present rules allow for a fine of Rs 25 for anyone spitting in public, the PMC has decided to step up the amount to a whopping Rs 1,000.

The move, prompted by the spread of the swine flu virus, will take effect from Saturday itself. The existing nuisance detection squads of each ward office are expected to take action against spitters.

Employer-backed health insurance not enough:

"Please don't rely on your employer to take care of your health, it is we who need to take utmost precaution as it our life that is at stake. We have the opportunity to take care of our health related expenses by paying a very minimum sum as premium."

Typical expenses that are not covered by a general health insurance policy:

Any disease or injury within the first 30 days of commencing a policy, except in the case of accidental injury. Any treatment, relating to any illness or disease already in existence at the time of the insurance proposal. These are called pre-existing conditions.

There is also a whole list of diseases and disorders where expenses incurred within the first two years from the start date of the policy are not covered. These diseases are typically covered from the third or fourth year onwards as pre-existing covers. Most sellers of health insurance will never tell you about this.

Take the health insurance policy at the earliest so that all diseases are comprehensively covered under the policy. Once we continue to renew the policy for three or more years without any claim, it becomes documentary evidence that any kind of disease can be reimbursed.

This is a very strong legal point which needs to be kept in mind in case the company does not agree to reimburse the cost citing odd reasons.

Why you need to have an individual health policy though you are covered under corporate health policy?

The corporate health policy ceases under following circumstances:

The company winds up.
You resign or you are terminated from the company.
You retire from the services at the age of 60.
The company decides to withdraw the health policy benefit to employees, i.e. there is no law that the company should provide health policy to employee.
Most corporate policies also have restrictive covers.

In all the above situations you will face severe problems if you don't hold an individual health policy. One very important point we need to understand is that after the age of 45, no insurance company will provide a health policy without medical check examination.

Once the medicals are done and the individual is prone to even 0.001% of any disease, that particular ailment will be excluded from the policy and he/she cannot claim the expense

Coming soon: Swine flu vaccine made in India

India may soon get its own vaccine for swine flu, or the H1N1 virus, that has killed hundreds across the globe this year. Three domestic biotech firms — Serum Institute, Bharat Biotech and Panacea Biotech — have submitted their plans to launch the vaccine by March 2010, an official in the health ministry said.

The companies have already started the preliminary research and they have submitted a detailed timeline for developing the vaccine to the drug regulator, he said, requesting anonymity.

The panic continues even though by now we have all been told many times over that the swine flu is a mild disease with a low rate of mortality. We have seen the evidence of other countries and heard the testimony of those who have been through it themselves. And we know that the number of fatalities nationally is currently lower than that from a bus rolling into a nullah, something that occurs every other day in the hills.

Some of the reasons for this continued fear are easy to come by.

The epidemic could balloon into something much larger and given that there appears to be no vaccine on the horizon, this understandably breeds uncertainty.

Also there is an element of randomness that makes anybody a potential victim and this is unnerving. The fact that it is transmitted by air makes us distrust what is otherwise the most natural and unconscious act we perform – that of breathing.

Unlike some other public health tragedies that are linked to social and economic class and are thus seen as someone else’s problems, no such immunity is available here.

And yet, in a country where every year dengue claims so many lives and yet the mosquito problem is treated with such indifference that the government has to plead with its citizen to perform simple acts of sanitation like cleaning air coolers in their own homes to keep mosquitoes away, it does seem odd that another disease should ignite this much fear

Health ministry issues fresh guidelines on swine flu

With swine flu cases continuing to rise sharply, the Union Health Ministry on Saturday issued fresh guidelines specifying screening and testing module for patients to contain the disease in the country:

As per guidelines, all individuals seeking consultation for flu-like symptoms would be screened at healthcare facilities -- both government and private -- or examined by a doctor and will be categorised broadly under three categories A, B and C depending on their health status, as per an official release here.

They will be given treatment and isolated as specified in the guidelines.
Patients with mild fever plus cough/sore throat with or without body ache, headache, diarrhoea and vomiting will be categorised as Category-A. They do not require Oseltamivir and should be treated for the symptoms mentioned above.

The patients should be monitored for their progress and reassessed after 24 to 48 hours by the doctor. For this category, no testing of the patient for H1N1 is required. Patients should confine themselves at home and avoid mixing up with public and high risk members in the family.

Category B has two subcategories. Category BI is for those patients, who have high grade fever and severe sore throat in addition to all the signs and symptoms mentioned under Category-A. They may require home isolation and Oseltamivir.

Category BII is for those, who have in addition to all the signs and symptoms mentioned under Category-A are having one or more of the following high risk conditions like children less than 5-years-old, pregnant women, persons aged 65 years or older, patients with lung diseases, heart disease, liver disease, kidney disease, blood disorders, diabetes, neurological disorders, cancer and HIV/AIDS, patients on long term cortisone therapy. They shall be treated with Oseltamivir but no tests for H1N1 is required for Category-B (I) and (II).

However, all patients of Category-B (I) and (II) should confine themselves at home and avoid mixing with public and high risk members in the family.

In category C, those patients fall, who in addition to the above signs and symptoms of Category-A and B, have one or more of the following problems like breathlessness, chest pain, drowsiness, fall in blood pressure, sputum mixed with blood, bluish discolouration of nails, irritability among small children, refusal to accept food, worsening of underlying chronic conditions.

All these patients mentioned above in Category-C require testing, immediate hospitalisation and treatment.

Swine flu: Learn from ’94 plague - S A Aiyar

Swine flu has killed 21Indians so far, and may soon claim hundreds, even thousands. That is a tragedy. Even so, swine flu remains a very minor cause of death, far behind other diseases that kill millions. The panic generated by the media is unwarranted, and is worsening health outcomes.

In 2001-03, the Registrar General conducted a survey to gauge the main causes of deaths in India. Heart disease came first (19%), followed by respiratory diseases like asthma (9%), diarrhea (8%), respiratory infections like pneumonia (6.2%), tuberculosis (6%), and cancer (5.7%).

Applying these percentages to India’s annual deaths of around 9 million, we find that 1.37 million people die annually of respiratory diseases and infections, 7,20,000 of diarrhea, and 5,40,000 of tuberculosis. These are staggering numbers. They imply that on an average day, 3,753 people die of respiratory diseases and infections, 1,973 of diarrhea, and 1,479 of tuberculosis.

Seen in this light, 20-odd swine flu deaths are almost laughably trivial. I do not laugh, because every death is a tragedy. But i am infinitely sadder for the millions whose plight has been swept out of public view, and is actually being worsened by upper-class panic.

Make no mistake, swine flu panic is substantially an upper class worry. Why do the media overflow with news of swine flu while ignoring other diseases that kill thousands every day? Because those everyday diseases are the problems of the poorer half of India, and the media target the upper half. Some upper class folk do get asthma or TB, but they are quickly treated and rarely die of these diseases. The millions who die come from the bottom half, lacking access to doctors and medicines. They die so regularly in millions that their deaths are no longer considered news.

2 Naidu docs test +ve

Two doctors and a nurse at the Naidu hospital tested positive for the H1N1 virus on Friday while another two doctors have been home-quarantined as suspected cases.

Reacting to this, doctors at the hospital alleged that the authorities have failed to provide them with basic precautionary gear.

Chief minister for flu review before schools reopen

Chief minister Ashok Chavan on Sunday said that the swine flu situation in Pune had “stabilized” and was “under control” and that a decision regarding the reopening of schools, colleges and other commercial establishments would be taken by the district guardian minister and the local administration.

Speaking to reporters here after holding a meeting with the civic and district officials, Chavan said it would take some time for the situation to be brought fully under control.

Chavan said the guardian minister and the local administration would review the situation before taking a decision to reopen schools and colleges. “A decision will be taken at least 24 hours before the date that was earlier announced for reopening schools and colleges,” he added."

A 36-year-old woman died of swine flu in Sasson hospital in Pune late on Sunday night, official sources said.

The woman named Bina Gonsalves was admitted to the hospital on August 13 with suspected swine flu symptoms, they said.

In retrospect, the worst thing about having an angioplasty is being wheeled into the Operation Theatre. There you are, a perfectly normal human being, walking into the hospital vertically, and a little while later, shorn of your own clothes and some body hair, you are horizontal on a gurney, staring at the Bombay Hospital corridors' ceiling, then the lift ceiling, then the operation theatre ceiling, all the while avoiding the eyes of those staring at you. (Why does everyone stare uninhibitedly at someone on a stretcher? I suppose because you are breaking the rule of vertical motion).

Citizens cry foul as politicians shy away from duty

Civic activists have alleged that political parties are shying away from taking active part in supporting the administration in creating public awareness about the H1N1 flu in the city. However, party leaders, across the line, claim that they have made all efforts they could.

Vivek Velankar of Sajag Nagrik Manch says that city politicians always want to be "politically correct". "The annual Ganesh festival is approaching. On the backdrop of swine flu outbreak, the central and state government have issued guidelines against public gatherings. Political parties don't want to take any stand on the festivals as it is a sensitive issue. No political party would like to disturb its vote bank ahead of elections," Velankar added.

Meanehile, in a statement issued to the press on Sunday, city MP Suresh Kalmadi appealed to citizens not to panic saying that the H1N1 flu can be cured if it is diagnosis and treated on time. Citizens should visit the designated government hospitals for screening, he added.
